Client refers to the individual, power of attorney, or family member/guardian in charge seeking placement. Facility refers to Assisted Living residences/care homes.

Release of Information: In order to obtain the finest possible long-term care option, a CALLRN specialist will need to collect information regarding the individual seeking our services. Facts such as age, gender, care requirements, mental status, health history, and payment information are required. The client understands and authorizes CALLRN to utilize such information for long-term care option purposes only. CallRN follows and upholds privacy provisions as defined by HIPPA laws.

In order for our referral services to remain at NO COST and avoid conflict with other agencies, you agree to work with only one referral agency.

Please contact CALLRN prior to contacting any facilities.

Our Nursing Case Management fees are waived for those clients placed through our services, which includes initial assessment, tours, and follow-up visits. Clients, who choose a facility on their own, without consulting their RN Case Manager, will not qualify for the fee waiver and/or our Nursing services. Therefore, it is imperative that the client inform us regarding a facility not discussed prior to contacting that facility. Based on services, our RN Case Management Rates vary, up to $80 per hour.

By requesting a facility search, you, the client, authorize a CALLRN specialist to seek long-term care options on your behalf; however, the ultimate decision to place or be placed in a particular facility or choose in- home care resources is at your sole discretion.
The Facilities selected by meet the State Department of Health Services licensing requirements.

Selection of Facility for Client: In general, facilities are chosen according to various criteria including, but not limited to: financial cost, geography, home characteristics, special features, and client preferences.

Fees and Payment: Our agency provides a complimentary referral service to clients. Typically our fees are paid by the facility for services provided. Fees for web advertising are paid by facilities and advertisers, CallRN has no liability with or for the facilities listed on the database.

It is ultimately the client's responsibility to assess and decide on the most suitable facility before placement. It is the facility's responsibility to assess and decide on the client before placement. Once a contractual agreement has been obtained between the client and facility, CALLRN or any individual working for CALLRN shall not be held liable for any negative occurrences, negligence, recklessness, or incompatibility concerning the client while under the care of the facility.

In the unlikely event that either the facility or the client determines incompatibility, CALLRN will be notified to determine alternative placement.
